Social Worker - Community Living Center
This social worker is assigned to the Community Living Center (CLC) at the Canandaigua VA Medical Center.
This social worker is responsible for providing social work services to the veteran residents of this area.
The incumbent is a member of an interdisciplinary team, and is primarily responsible for completing psychosocial history and assessments, completing the psychosocial portions of the care plan and the Minimum Data Set (MDS).
The social worker is also responsible for facilitating the Interdisciplinary Treatment Team meetings, emphasizing culture transformation, including resident centered care planning.
The CLC social worker may also be responsible for participating in annual inspections of VA Contract Nursing Home facilities as well as clinical eligibility assessments for the Caregiver Support program.
The incumbent is assigned to the Social Work Service and reports to the Chief, Social Work Service.
He/she is assigned to the VA Medical Center and provides comprehensive social work services to veterans and their families.
Responsibilities include the management, coordination and provision of social work services individually and/or in groups to Veterans and their families in inpatient, outpatient and community settings.
In the provision of care in patients from the adult (less than 65 years of age) to the geriatric (greater than 65 years of age), the employee recognizes the age-related needs and demonstrates knowledge and care appropriate to that age group.
In the performance of official duties, the incumbent has regular access to printed and electronic files containing sensitive data which must be protected under provision of the Privacy Act of 1974 and other applicable laws, Federal Regulations, VA statues and policy.
Incumbent is responsible for protecting data from unauthorized release of access codes, etc.
