The Nursing Home Placement Coordinator provides a centralized resource for all Nursing Home referral and discharge activities.
The BSW/BS role is essential for the communication and planning for post discharge care in a Nursing Home setting.
Excellent communication with unit interdisciplinary teams and Nursing Home Admissions teams is critical to our hospital throughput.
Education:
Required - Bachelor's Degree Preferred - BSW
Experience:
Required - no previous experience required Preferred - at least 2-3 years in a health care setting, preferably either an acute hospital setting or skilled nursing facility setting Thompson Health is an EOE encouraging women, minorities, individuals with disabilities, and veterans to apply.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
